- Patient, carer, and healthcare professional education and training programs.

We understand that it is better for patients to see the same nurse, allowing relationships to be built and the reassurance of a familiar point of contact, we facilitate this wherever possible. We are also able to provide out-of-hours telephone support for added peace of mind.
All of our highly experienced nurses satisfy strict regulatory requirements and are Disclosure and Baring Service (DBS) checked and are registered with the Nursing and Midwifery Council (NMC). We also ensure continued personal and professional development and regular clinical competency audits and reviews for all of our nurses.
Patient support programs
Pharmaxo Healthcare has experience in the design and delivery of patient support programmes on behalf of pharmaceutical industry clients. Patients’ failure to adhere to their medication as prescribed (i.e. taking the correct doses at the appropriate intervals for the full duration of treatment) is well recognised. Our adherence programmes aim to maximise the likelihood that medicines will be taken as the healthcare professional intended, through a tailored mix of various interventions as well as providing patient education and support.
Examples of some of the interventions we deliver include:
- Reminders and access to information to overcome forgetfulness, confusion, and misunderstandings.
- Patient education to support understanding of their illness and treatment regime.
- Helping to manage patients as they prepare for treatment e.g. coping with side effects and practical advice to minimise their occurrence.
- Listening to patient concerns and talking through barriers to adherence.
Confidentiality and data protection
At Pharmaxo Healthcare, we take information governance extremely seriously. The data that we capture and record is regulated and controlled so that as a trusted home healthcare provider we retain the trust and confidence of our customer.
We are trusted by physicians, hospitals, and patients to safeguard sensitive information and we must ensure all data held is treated respectfully and lawfully. Under the provisions of the Data Protection Act 1998, Pharmaxo Healthcare are required by law to ask for the patients’ permission to maintain and use personal information regarding them and their health.
The information held by Pharmaxo Healthcare relating to our patients may refer to a medical condition, medication received, etc. This information is only for use in order to facilitate the supply of the medication and to advise on its use. The details are held securely and this information is not made available to third parties unless those specifically involved in the provision of the therapy.
Patient consent is sought directly from the patient at the beginning of the process of registering them to receive clinical home care from Pharmaxo Healthcare. This is completed via the relevant section on our patient registration and consent form.
Pharmaxo Healthcare has a dedicated Caldicott Guardian; a senior person responsible for protecting the confidentiality of patient and service-user information and enabling appropriate information sharing. Acting as the “conscience” of an organisation, our Caldicott guardian actively supports work to enable information sharing where it is appropriate to share and advises on options for lawful and ethical processing of information.
As part of a commitment to patient confidentiality, all Pharmaxo Healthcare staff are screened using the Disclosure and Barring Service so that our customers and patients can be reassured and so that they have trust in our service.
A breach of confidentiality is extremely rare, however, if they do occur, our process of identifying and capturing these incidents is equally robust. All incidents are reported and logged via our internal deviation management process and then investigated prior to review and closure by our Senior Management Team.
Clinical governance and patient safety
Strong clinical governance ensures that Pharmaxo Healthcare delivers care of the highest standards of safety and quality to your patients. This ethos underpins our organisation’s culture, systems and the working practices of our staff.
We have a governance team, chaired by our Superintendent Pharmacist and Quality Lead, which meets regularly to discuss and improve the service we offer to our patients. Our rigorous quality control and clinical governance procedures ensure that all our patients receive the best and safest standards of care.
The team will ensure core processes are in place to produce the desired clinical outcomes, assess and manage risks, monitor performance on key indicators and decide upon corrective measure as appropriate. It oversees the work of all departments within the company, that includes:
- Risk Management
- Quality Assurance
- Regulatory requirements
- Audit
- Information management
- Recruitment, education, and training
Pharmaxo Healthcare has processes in place in the event of the need for a product recall, allowing all inventory supplied or held in stock by the business to be identified, recovered, quarantined and destroyed if required. Pharmaxo Pharmacy Services receives notification and alerts from the MHRA and as a result receives instant information on product recalls if made by the product license holder.
As a contracted extension of the pharmaceutical company, Pharmaxo Healthcare is required to report any adverse drug event that comes to our attention. As a healthcare organisation, we have a professional and ethical obligation to report ADEs, even when a contract is not in place with a pharmaceutical company.
Adverse drug events are reported as soon as we become aware and usually specify within 24 hours. All Pharmaxo Healthcare staff receive mandatory training on how to recognise, respond to and report adverse drug events. This includes our nurses, drivers and patient services teams and covers training on the MHRA Yellow Card Scheme.
Pharmaxo Healthcare also has a lone worker procedure for ensuring a safe environment for the provision of care. This includes training of our patient-facing staff such as nurses as well as undertaking processes such as risk assessments to minimise the risk for lone workers.
The Pharmaxo Healthcare patient journey
Pharmaxo Healthcare aims to make the process of registering and referring patients onto our services as straightforward and efficient as possible. Our patient services team are on hand to manage the whole process.
Registration and referral
At all times our services are underpinned by the Pharmaxo Healthcare patient services team. This team is the primary point of contact for our patients. Prescription management and dispensing
Pharmaxo Healthcare has a fully equipped pharmacy, staffed by a team of highly trained pharmacists, nurses and patient services advisors, which has the capacity to handle thousands of items every month, providing significant efficiency savings for our NHS customers.
Our pharmacy team is on hand to ensure that the right medicines are dispensed at the right doses to the right patients and, where appropriate, give advice. Upon receiving a home care patient prescription from a referring trust or clinic, this is assessed by a Pharmaxo Healthcare pharmacist.
Prescriptions are only dispensed by trained and qualified dispensers before being accuracy checked by a pharmacist. If at any time there are questions or queries involving the patient or their prescription, the referring hospital /clinic will be contacted by our prescription management team to resolve them.
Similarly, at the agreed time before a prescription expires, the referring hospital will be contacted by the Pharmaxo Healthcare team who will then request new prescriptions for patients as appropriate.
Compounded medicines can also be supplied by Pharmaxo Healthcare. We work with our sister company, Bath ASU, to ensure that we have the capability and the infrastructure to provide aseptic solutions which meet our customers’ needs.
Pharmaxo Healthcare can dispense and provide ancillaries and medical equipment used in the administration of home healthcare services, such as syringes, infusion devices, and pumps.
