Pharmaxo Healthcare complies with the requirements of the Data Protection Act 2018, meaning you can be assured that your personal information is kept strictly confidential. Your details are only available to the specific Pharmaxo Pharmacy Services employees involved in the provision of your home care service.

Under the provisions of the Data Protection Act 2018, Pharmaxo Healthcare are required by law to have a valid legal basis for using personal information about a patient and their health.

The information about you that we hold may refer to a medical condition, medication received, etc. This information is only used in order to aid the supply of medication and to advise on its use. The details are held securely by Pharmaxo Healthcare and this information is not made available to third parties unless those are specifically involved in the provision of the therapy.

Where it is relevant for us to rely on the valid legal basis of consent, your consent is sought at the beginning of the Pharmaxo Healthcare home care referral process. Your permission is sought using the patient registration and consent form, which will be completed with you in conjunction with your referring clinician or trust.

As part of a commitment to patient confidentiality, all Pharmaxo Healthcare staff are screened using the Disclosure and Barring Service so that our customers and patients can be reassured and have trust in our service.

As a patient receiving services from Pharmaxo Healthcare, should you wish to access and view the information held on you, please request and submit a ‘Request for Information’ form to us. Our patient services team will be able to provide this form if required.

Additionally, from time to time, we may ask you to complete a service questionnaire. The purpose of this is to help us make sure we are providing you with the service you expect and to give us information on any improvements you feel necessary. This will always be done with the permission of your hospital or clinic and your answers are kept anonymous and confidential.
